Postpartum Heaviness: When It's Normal Healing and When It's Prolapse

After delivery, many women feel a dragging or heavy sensation in the vagina. In the first few weeks this is usually normal, because the tissue and ligaments supporting the pelvic organs stretched during birth and need time to recover.

What separates ordinary postpartum healing from pelvic organ prolapse is timing and pattern. Heaviness that gradually eases week by week generally reflects normal recovery. Heaviness that worsens with standing, lifting, or as the day goes on — especially alongside a sense of a bulge or pressure in the vagina — can point to a pelvic organ sitting lower than it should.

Other things worth noticing alongside it: a feeling of incomplete bladder emptying, needing to change position to finish urinating, or something you can feel or see at the vaginal opening when straining or coughing. These do not always resolve on their own, and pelvic floor training started in the postpartum period tends to help both ordinary recovery and early prolapse alike.

A postpartum pelvic floor assessment can tell the difference clearly — between normal healing in progress and early signs of prolapse that would benefit from monitoring or more targeted muscle training. You do not need to wait until symptoms are severe before being assessed.
