A feeling of pressure, a soft bulge you can sense or even touch, a dragging heaviness that is worse at the end of the day and better after lying down. Many women notice this quietly and say nothing for months, assuming it is something to put up with. It is common, and in most cases it can be assessed and managed.
The usual explanation is pelvic organ prolapse: the bladder, womb or back wall of the vagina settling lower than before because the supporting muscles and connective tissue have loosened. Childbirth, years of heavy lifting, chronic constipation and the hormonal changes around menopause all contribute. Other things can feel similar, including an over-tight pelvic floor, a vaginal cyst or a polyp, so it is worth having it looked at rather than guessing.
A few things are sensible to do first. Notice when it is worse, whether you also have leaking, a sense of incomplete emptying or difficulty with bowel movements, and whether lifting or coughing changes it. Avoid straining on the toilet, and hold off on new heavy lifting until you have been checked. None of this causes harm to wait a few days for an appointment, but new bleeding, an inability to pass urine or severe pain should be seen promptly.
Assessment is a calm, private examination, usually done lying down and sometimes standing or with a cough, because a prolapse can look very different in different positions. We describe which structure is involved and how far it descends, then discuss options in order of invasiveness: pelvic floor training that is taught correctly, lifestyle adjustments, a supportive pessary where suitable, and only then surgical opinions if they are needed.
Many women with mild descent need no procedure at all, and finding out early simply gives you more choices. Being told what is actually happening is often the biggest relief.
