ต้องใช้นิ้วช่วยจึงถ่ายได้หมด: ทำความรู้จักผนังช่องคลอดด้านหลังหย่อน

ผู้หญิงจำนวนไม่น้อยใช้นิ้วกดที่ช่องคลอดหรือบริเวณฝีเย็บเพื่อให้ถ่ายได้หมด แล้วไม่เคยเล่าให้ใครฟัง เพราะคิดว่าเป็นเรื่องเฉพาะตัว ในทางการแพทย์เรียกพฤติกรรมนี้ว่า splinting และมักเป็นสัญญาณของภาวะผนังช่องคลอดด้านหลังหย่อน หรือ rectocele

กลไกคือ ผนังกั้นระหว่างลำไส้ตรงกับช่องคลอดอ่อนแรงลง ลำไส้ตรงจึงโป่งมาทางด้านหน้าเป็นกระเปาะ อุจจาระตกลงไปค้างในกระเปาะนั้นแทนที่จะเคลื่อนตรงออกมา ยิ่งเบ่งแรงยิ่งดันเข้าไปในกระเปาะ ไม่ใช่ดันออก การใช้นิ้วกดจึงได้ผล เพราะเป็นการเปลี่ยนมุมและบีบกระเปาะให้ว่าง

อาการที่มักมาด้วยกัน ได้แก่ รู้สึกหน่วงถ่วงในช่องคลอดที่แย่ลงตอนเย็น ต้องเปลี่ยนท่าเพื่อถ่ายให้หมด รู้สึกว่ายังมีอุจจาระค้าง และบางรายมีปัสสาวะไม่สุดร่วมด้วยเพราะผนังด้านหน้าหย่อนไปพร้อมกัน อาการมักเบาที่สุดตอนเช้าและหนักขึ้นเมื่อยืนมาทั้งวัน

การรักษาไม่ได้เริ่มที่การผ่าตัด ระดับที่ไม่รุนแรงมักเริ่มจากการทำให้อุจจาระนุ่มพอที่จะไม่ต้องเบ่ง ปรับท่านั่งถ่าย ฝึกกล้ามเนื้ออุ้งเชิงกรานอย่างถูกวิธี และห่วงพยุงช่องคลอดในผู้ที่เหมาะสม การผ่าตัดพิจารณาเมื่ออาการรบกวนชีวิตประจำวันหรือวิธีอื่นไม่ได้ผล

ควรพบแพทย์เมื่อ ต้องใช้นิ้วช่วยทุกครั้ง คลำได้ก้อนที่ปากช่องคลอด มีเลือดออก ปวดขณะถ่ายหรือขณะมีเพศสัมพันธ์ หรือมีปัสสาวะติดเชื้อซ้ำ ๆ ทั้งหมดนี้ประเมินได้ในการตรวจภายในครั้งเดียว

Needing to Press with a Finger to Empty: Understanding Rectocele

A significant number of women press against the vagina or perineum to finish a bowel movement and never mention it to anyone, assuming it is a personal quirk. The clinical term is splinting, and it usually points to a prolapse of the back vaginal wall, known as a rectocele.

The mechanism is straightforward. The wall between the rectum and the vagina weakens, so the rectum bulges forward into a pocket. Stool settles into that pocket instead of passing straight through, and pushing harder drives it further into the pocket rather than out. Pressing with a finger works because it changes the angle and empties the pocket manually.

Associated symptoms include a dragging or heavy sensation in the vagina that worsens through the day, needing to change position to finish, a persistent feeling of incomplete evacuation, and sometimes incomplete bladder emptying as well, because the front wall can descend at the same time. Symptoms are typically mildest in the morning and worst after a day on your feet.

Treatment does not begin with surgery. Milder cases are usually managed by keeping stool soft enough that straining is unnecessary, changing toilet posture, correctly taught pelvic floor training, and a vaginal pessary where it suits. Surgery is considered when symptoms interfere with daily life or when other approaches have not worked.

See a doctor if you need to splint every time, can feel a bulge at the vaginal opening, have bleeding, experience pain on defecation or during intercourse, or have repeated urinary infections. All of this can be assessed in a single pelvic examination.
