Hormone testing: when it helps, and when it cannot

Hot flushes, broken sleep, mood changes, irregular periods — and the reasonable expectation that a blood test will settle the question. Sometimes it does. Often it cannot, and knowing which situation you are in saves both money and frustration.

Why one blood test often cannot answer it

Perimenopausal hormones fluctuate enormously — not only month to month but week to week. A single FSH or estradiol reading is a snapshot of a moving target.

So a normal result, taken on a day when levels happen to be normal, does not rule out perimenopause. That is the single most common misunderstanding about this test, and it sends women away reassured that nothing is happening when something is.

This is why guidelines generally say that in women over 45 the diagnosis is made on symptoms and cycle pattern rather than on blood results.

When testing genuinely changes the answer

Worth doing

Under 40. Premature ovarian insufficiency has to be distinguished, and the result changes management substantially. Here testing is not optional.

Between 40 and 45. The result adds information to a picture that is otherwise ambiguous.

When something else could explain the symptoms — particularly thyroid disease and anaemia, both of which cause fatigue and mood change, both common in this age group, and both treatable. This is often the most valuable test of the set and it is not a sex-hormone test at all.

When a specific decision depends on the number. If nothing about your management would change either way, the test is costing you money to confirm what the history already says.

What is more useful than a test

A record of symptoms and cycles. The dates of your recent periods, what changed and when, how you are sleeping, and how each symptom behaves across the month. That tells a doctor more than one reading taken on an arbitrary day.

It costs nothing, it can be started today, and it is the thing most women arrive without.

Also worth separating: which symptoms are hormonal and which are not. Vaginal dryness, urinary urgency and discomfort with sex are frequently hormonal and frequently treatable — and frequently go unmentioned, because they are assumed to be a separate problem or not important enough to raise.

What the appointment involves

The history first, because in most cases it is what decides the diagnosis. Then the tests that are actually indicated for your age and picture, rather than a fixed panel.

You will be told what was found, what it does and does not establish, and which options apply — including non-hormonal ones, and including deciding to do nothing for now.

Suitability for any treatment is decided individually, and results vary between individuals.

Consultation ฿1,500–3,000 depending on complexity, plus the ฿300 clinic service fee. Any tests are quoted before they are done.

When to be seen

If periods stopped before 45. If bleeding is heavy, or happening between periods. If symptoms are interfering with work or sleep. Or if you simply want to discuss the options, including the non-hormonal ones.

Bleeding after menopause is never part of the normal picture and should be assessed promptly rather than watched.

Heavy perimenopausal bleeding is worth raising even if you have decided it is normal for you — it is a common cause of iron deficiency, and the fatigue that follows is often attributed to menopause instead.
