ตรวจอุ้งเชิงกรานครั้งแรก แพทย์ทำอะไรบ้าง

ผู้หญิงจำนวนมากเลื่อนการตรวจอุ้งเชิงกรานออกไปนานหลายปี ไม่ใช่เพราะอาการไม่รบกวน แต่เพราะนึกภาพไม่ออกว่าการนัดตรวจครั้งแรกจะเป็นอย่างไร ความจริงแล้วการตรวจเริ่มต้นด้วยสิ่งที่ไม่น่ากลัวเลย นั่นคือการพูดคุย นั่งสบาย ๆ ในห้องตรวจส่วนตัว โดยยังสวมเสื้อผ้าตามปกติ จะไม่มีการตรวจร่างกายใด ๆ ก่อนที่คุณจะได้เล่าด้วยคำพูดของตัวเองว่าเกิดอะไรขึ้นบ้าง

การพูดคุยนี้คือส่วนที่สำคัญที่สุด แพทย์จะถามถึงอาการปัสสาวะเล็ดและสิ่งที่กระตุ้นให้เกิด ความรู้สึกปวดปัสสาวะเร่งด่วน ความถี่ในการเข้าห้องน้ำ การตื่นกลางดึก ความรู้สึกหน่วงถ่วง อาการทางลำไส้ และความไม่สบายขณะมีเพศสัมพันธ์ จะถามถึงประวัติการคลอด ประวัติการผ่าตัด วัยหมดประจำเดือน และที่สำคัญที่สุดคืออาการใดรบกวนคุณมากที่สุดและเกิดขึ้นเมื่อไร เพราะแผนการดูแลจะสร้างขึ้นจากจุดนั้น นอกจากนี้อาจขอให้คุณจดบันทึกการปัสสาวะและอาการต่อเนื่องสองถึงสามวัน เพราะความทรงจำมักกลบรายละเอียด ขณะที่บันทึกตามจริงเพียงไม่กี่วันจะเผยรูปแบบที่มองไม่เห็นด้วยวิธีอื่น

หากจำเป็นต้องตรวจร่างกาย แพทย์จะอธิบายทีละขั้นตอนก่อนเริ่มเสมอ ว่าจะตรวจอะไร เพื่ออะไร และจะรู้สึกอย่างไร การขอความยินยอมจะทำในทุกขั้นตอน ไม่ใช่เพียงครั้งเดียวตอนต้น โดยทั่วไปจะเริ่มจากการดูภายนอก สังเกตการทำงานของกล้ามเนื้ออุ้งเชิงกรานขณะขมิบ ผ่อนคลาย และไอ ส่วนการตรวจภายในจะให้ข้อมูลเรื่องความแข็งแรง ความทนทาน และการประสานงานของกล้ามเนื้อได้ละเอียดกว่ามาก แต่เป็นสิ่งที่แพทย์เสนอให้เลือก ไม่ใช่ถือว่าต้องทำ คุณปฏิเสธได้ หรือขอให้หยุดเมื่อไรก็ได้ โดยไม่ต้องอธิบายเหตุผล ที่ THE FIT CLINIC คุณสามารถพาคนที่ไว้ใจมาด้วย หรือจะค่อย ๆ ตรวจทีละส่วนก็ได้

สิ่งที่การตรวจต้องการหาคำตอบคือ กล้ามเนื้ออุ้งเชิงกรานอ่อนแรง ตึงตัวมากเกินไป หรือทำงานไม่ประสานกัน กล้ามเนื้อที่อ่อนแรงจะเกร็งค้างไว้ไม่ได้ กล้ามเนื้อที่ตึงเกินไปจะอยู่ในภาวะเกร็งตลอดเวลา ทำให้ปวดปัสสาวะเร่งด่วน เจ็บ และถ่ายปัสสาวะไม่สุด ซึ่งการขมิบแบบ Kegel มักทำให้แย่ลง ส่วนกล้ามเนื้อที่ทำงานไม่ประสานกันอาจแข็งแรงดี แต่หดตัวช้าไปหรือคลายตัวผิดจังหวะ ทั้งสามภาวะนี้ดูคล้ายกันจากภายนอก แต่ต้องการการดูแลที่ต่างกันมาก จึงเป็นเหตุผลว่าทำไมการเดาจากอาการเพียงอย่างเดียวจึงมักไม่ได้ผลอย่างที่หวัง

What Actually Happens During a Pelvic Floor Assessment

Many women put off a pelvic floor assessment for years, not because their symptoms are trivial but because they cannot picture the visit. It begins with nothing more intimidating than a conversation, sitting down, fully dressed, in a private room. Nothing is examined before you have explained, in your own words, what has been happening.

That conversation matters most. Your doctor will ask about leaking and what triggers it, urgency, daytime frequency and waking at night, heaviness, bowel symptoms and discomfort during intimacy. She will ask about your births, surgery and menopause, and above all what bothers you most and when, since the plan is built around that. You may also be asked to keep a bladder and symptom diary for a few days: memory smooths things over, whereas honest recording reveals patterns neither of you would see.

If a physical assessment is appropriate, it is explained step by step first: what will be looked at, why, and how it will feel. Consent is asked at each stage, not once at the start. It usually begins externally, watching how the pelvic floor moves as you tighten, relax and cough. An internal examination tells us much more about strength, endurance and coordination, but it is offered rather than assumed: you may decline, or ask to stop at any moment. We are looking at whether the pelvic floor is weak, overactive or poorly coordinated. A weak floor cannot hold a contraction; an overactive one sits in constant tension, causing urgency, pain and difficulty emptying, and Kegels usually make it worse; a poorly coordinated floor may be strong yet contract late or release at the wrong moment. They look alike from outside yet need quite different treatment, which is why guessing from symptoms alone so often disappoints.
