ยอดช่องคลอดหย่อนหลังผ่าตัดมดลูก: ทำไมยังเกิดขึ้นได้ทั้งที่ไม่มีมดลูกแล้ว

หลายคนแปลกใจว่าทำไมภาวะหย่อนยังเกิดขึ้นได้หลังผ่าตัดมดลูกออกไปแล้ว ในเมื่อไม่มีมดลูกเหลืออยู่ให้หย่อนลงมา สิ่งที่ยังหย่อนลงมาได้คือยอดช่องคลอด (vaginal vault) ซึ่งเป็นส่วนปลายสุดของช่องคลอดที่เคยเชื่อมกับมดลูก และรองรับด้วยเอ็นและเนื้อเยื่อเกี่ยวพันชุดเดียวกับที่เคยรองรับมดลูก หากการผ่าตัดมดลูกเกิดขึ้นเพราะภาวะหย่อนอยู่แล้ว หรือโครงสร้างรองรับเหล่านี้อ่อนแรงอยู่ก่อนจากการคลอดบุตรหรือการเบ่งเรื้อรัง ความเสี่ยงนั้นก็ยังคงอยู่ต่อไป ไม่ได้หายไปพร้อมกับการผ่าตัด

ยอดช่องคลอดหย่อนมักปรากฏช้ากว่าภาวะหย่อนชนิดอื่น บางครั้งหลายปีหลังผ่าตัด เพราะเกิดขึ้นอย่างค่อยเป็นค่อยไปจากแรงกดดันในชีวิตประจำวัน เช่น การไอ การเบ่ง การยกของหนัก หรือการเปลี่ยนแปลงของเนื้อเยื่อในวัยหมดประจำเดือน ความรู้สึกจะคล้ายกับภาวะหย่อนชนิดอื่น คือความรู้สึกหน่วง มีก้อนยื่นออกมา หรือรู้สึกเหมือนมีอะไร "หลุดลงมา" ซึ่งมักสังเกตชัดขึ้นตอนเย็นหรือหลังยืนนาน ๆ

การประเมินทำเช่นเดียวกับภาวะหย่อนทั่วไป คือการตรวจร่างกายขณะเบ่งเบา ๆ เพราะยอดช่องคลอดที่อยู่ในตำแหน่งปกติตอนนอนราบอาจหย่อนลงมาชัดเจนเมื่อยืนหรือเบ่ง วิธีนี้บอกได้ว่าหย่อนลงมากเพียงใดและกระเพาะปัสสาวะหรือไส้ตรงเกี่ยวข้องด้วยหรือไม่ เพราะยอดช่องคลอดหย่อนมักเกิดร่วมกับกระเพาะปัสสาวะหย่อนหรือไส้ตรงหย่อน มากกว่าจะเกิดขึ้นเพียงลำพัง

แนวทางการดูแลก็คล้ายกับภาวะหย่อนชนิดอื่น คือการฝึกกล้ามเนื้ออุ้งเชิงกรานสำหรับรายที่เป็นน้อย ห่วงพยุงช่องคลอด (pessary) เป็นทางเลือกไม่ผ่าตัด หรือการผ่าตัดซ่อมแซมสำหรับรายที่เป็นมากหรือรบกวนชีวิตประจำวัน โดยวิธีผ่าตัดที่เหมาะสมขึ้นอยู่กับว่าเป็นการผ่าตัดซ่อมภาวะหย่อนครั้งแรกหรือเป็นซ้ำหลังผ่าตัดมดลูก หากคุณผ่าตัดมดลูกมาหลายปีแล้วและเริ่มสังเกตความรู้สึกหน่วงหรือมีก้อนยื่นออกมา เรื่องนี้ควรได้รับการตรวจประเมินอย่างจริงจัง ไม่ควรคิดไปเองว่าเป็นไปไม่ได้เพราะไม่มีมดลูกแล้ว

Vaginal Vault Prolapse After Hysterectomy: Why It Still Happens Without a Uterus

It surprises many women that prolapse can still happen after a hysterectomy — without a uterus, what is there left to fall? What can still descend is the vaginal vault: the closed-off top of the vagina where the uterus used to attach, held up by the same ligaments and connective tissue that supported the uterus itself. If the hysterectomy was performed because of, or alongside, a prolapse, or if those supporting structures were already weakened by childbirth or chronic straining, that risk carries forward rather than disappearing with the surgery.

Vault prolapse tends to show up later than other types — sometimes years after the original operation — because it develops gradually as the same support structures continue to stretch under everyday pressure such as coughing, straining, heavy lifting, or the tissue changes of menopause. The sensation is similar to other forms of prolapse: heaviness, a bulge, or a feeling that something has "dropped," often more noticeable by the end of the day or after standing for long periods.

Assessment works the same way as for any prolapse: a physical examination while you bear down, since a vault that sits normally lying down can descend noticeably on standing or straining. This shows how far it has moved and whether the bladder or rectum are also involved, since vault prolapse often accompanies a cystocele or rectocele rather than occurring on its own.

Management options mirror other types of prolapse — pelvic floor muscle training for milder cases, a vaginal pessary as a non-surgical support option, or surgical repair for more advanced or bothersome cases, with the specific approach depending on whether this would be a first prolapse repair or a recurrence after the earlier hysterectomy. If you had a hysterectomy years ago and are now noticing heaviness or a bulge, that is worth assessing on its own terms rather than assuming it cannot be prolapse because the uterus is already gone.
