Vaginal dryness, burning and painful sex are common after breast cancer treatment — whether from chemotherapy, from aromatase inhibitors, or from an early menopause brought on by treatment — and they are symptoms many women live with quietly because raising them feels like a separate, smaller problem next to the cancer itself. It is not smaller. It affects daily comfort, intimacy and confidence, and there are things worth discussing.

The reason this needs a careful conversation rather than a simple answer is that vaginal estrogen and systemic hormone replacement therapy are not the same thing, and the caution that applies to one does not automatically apply to the other. Systemic HRT raises estrogen levels throughout the body. Low-dose vaginal estrogen is applied locally and is absorbed into the bloodstream in far smaller amounts, which is why oncologists and gynecologists approach it differently — but "differently" does not mean the decision is simple or the same for everyone.

What is actually appropriate depends on specifics that only your oncology team can weigh properly: whether your cancer was hormone-receptor-positive or negative, what treatment you have had or are still on, how long ago you finished it, and your individual risk profile. This is precisely why the decision belongs jointly to your oncologist and a gynecologist familiar with post-cancer care, not to either specialty in isolation, and not to a general internet answer that treats all breast cancers as the same.
