หลังหมดประจำเดือน ผู้หญิงจำนวนมากมีอาการหลายอย่างพร้อมกันแล้วไปรักษาแยกกันทีละอย่าง ช่องคลอดแห้งไปหาหมอหนึ่งคน แสบเวลาปัสสาวะไปอีกที่ ติดเชื้อทางเดินปัสสาวะซ้ำ ๆ ก็ได้ยาปฏิชีวนะรอบแล้วรอบเล่า ทั้งที่ในหลายกรณีมันคือกลุ่มอาการเดียวกัน ซึ่งเกิดจากระดับเอสโตรเจนที่ลดลงส่งผลต่อเนื้อเยื่อช่องคลอด ท่อปัสสาวะ และกระเพาะปัสสาวะพร้อมกัน

อาการที่พบในกลุ่มนี้ ได้แก่ ช่องคลอดแห้ง แสบ คัน เจ็บขณะมีเพศสัมพันธ์ ปัสสาวะแสบขัดโดยที่เพาะเชื้อไม่ขึ้น ปวดปัสสาวะบ่อยหรือเร่งด่วน และการติดเชื้อทางเดินปัสสาวะที่กลับมาเป็นซ้ำบ่อยกว่าที่เคย ต่างจากอาการร้อนวูบวาบตรงที่กลุ่มอาการนี้ไม่หายไปเองเมื่อเวลาผ่านไป และมักค่อย ๆ แย่ลงถ้าไม่ได้รับการดูแล

สิ่งที่ควรรู้คือ เอสโตรเจนเฉพาะที่สำหรับช่องคลอดเป็นคนละเรื่องกับฮอร์โมนทดแทนแบบรับประทานทั้งระบบ ปริมาณที่ดูดซึมเข้ากระแสเลือดน้อยมาก และไม่ได้ถูกจัดอยู่ในความเสี่ยงกลุ่มเดียวกัน ผู้ที่มีข้อจำกัดทางการแพทย์บางอย่างยังมีทางเลือกอื่น เช่น สารหล่อลื่นและมอยส์เจอไรเซอร์สำหรับช่องคลอด ร่วมกับการดูแลอุ้งเชิงกราน แนวทางที่เหมาะสมขึ้นกับประวัติของแต่ละคนและควรตัดสินใจร่วมกับแพทย์

ควรพบแพทย์เมื่อ มีเลือดออกหลังหมดประจำเดือนซึ่งต้องตรวจเสมอ เจ็บขณะมีเพศสัมพันธ์จนหลีกเลี่ยง ปัสสาวะแสบเรื้อรัง ติดเชื้อทางเดินปัสสาวะมากกว่าสองถึงสามครั้งต่อปี หรือใช้ผลิตภัณฑ์เองแล้วไม่ดีขึ้น

After menopause many women develop several symptoms at once and then treat them separately. Dryness goes to one clinician, burning on urination to another, and repeated urinary infections get another course of antibiotics each time. In many cases these are a single condition, driven by falling oestrogen affecting vaginal, urethral and bladder tissue at the same time. It is described as genitourinary syndrome of menopause.

The cluster includes vaginal dryness, burning and itching, pain with intercourse, stinging on urination when cultures come back negative, urinary frequency or urgency, and urinary tract infections that recur more often than before. Unlike hot flushes, this cluster does not resolve on its own with time, and it tends to progress gradually when left unaddressed.

An important distinction: local vaginal oestrogen is a different treatment from systemic hormone therapy taken by mouth. Very little is absorbed into the bloodstream, and it is not grouped with the same risks. For women with specific medical contraindications there are other routes, including vaginal moisturisers and lubricants alongside pelvic floor care. Which approach suits you depends on your own history and should be decided with a doctor rather than from a headline.

See a doctor for any bleeding after menopause, which always needs assessment, for pain with intercourse that has led you to avoid it, for persistent burning on urination, for more than two or three urinary infections a year, or if over-the-counter products have not helped.
