ผลแปปสเมียร์ผิดปกติ: ขั้นตอนต่อไปคืออะไร

ไม่กี่ประโยคที่ทำให้ตกใจได้เท่ากับการรู้ว่าผลแปปสเมียร์ผิดปกติ และความตกใจส่วนใหญ่นั้นไม่จำเป็น มะเร็งปากมดลูกเป็นหนึ่งในไม่กี่มะเร็งที่การตรวจคัดกรองป้องกันได้จริง ไม่ใช่แค่พบเร็ว เพราะก่อนจะเป็นมะเร็งจะมีการเปลี่ยนแปลงที่รักษาได้อยู่นานหลายปี

ผลผิดปกติส่วนใหญ่หมายถึงการเปลี่ยนแปลงของเซลล์ ไม่ใช่มะเร็ง กลุ่มที่พบบ่อยไล่ตั้งแต่การเปลี่ยนแปลงเล็กน้อย ไปจนถึงการเปลี่ยนแปลงระดับต่ำซึ่งหายเองได้บ่อย และการเปลี่ยนแปลงระดับสูงซึ่งรักษาเพื่อไม่ให้ลุกลามต่อ มีเพียงส่วนน้อยเท่านั้นที่ใกล้เคียงกับมะเร็ง

ขั้นตอนต่อไปขึ้นกับระดับความผิดปกติและผล HPV ที่ตรวจร่วม ทางเลือกมีทั้งการตรวจซ้ำหลังเว้นระยะ เพราะการเปลี่ยนแปลงระดับต่ำจำนวนมากหายเอง การตรวจ HPV เพื่อประเมินความเสี่ยงให้ชัดขึ้น หรือการส่องกล้องตรวจปากมดลูก ซึ่งทำที่คลินิก ใช้เวลาราวสิบห้านาที และอาจตัดชิ้นเนื้อเล็กน้อยหากมีจุดที่ต้องตรวจเพิ่ม

หากต้องรักษา มักเป็นหัตถการเล็กเพื่อนำบริเวณที่ผิดปกติออก ทำแบบผู้ป่วยนอก ได้ผลดีมากและมีผลต่อการตั้งครรภ์ในอนาคตน้อย แต่ควรคุยล่วงหน้าหากวางแผนมีบุตร

สิ่งสำคัญที่สุดคือการไม่ข้ามการตรวจติดตาม การเปลี่ยนแปลงของปากมดลูกดำเนินไปช้า ซึ่งเป็นเหตุผลที่รักษาได้ดี และเป็นเหตุผลเดียวกันที่ทำให้เลื่อนนัดได้ง่าย แล้วเลื่อนอีก

ควรนำวันที่และผลการตรวจครั้งก่อนมาด้วย เพราะมีผลต่อการแปลผลจริง ๆ และควรพบแพทย์โดยเร็วหากเคยมีผลผิดปกติแล้วยังไม่ได้ตรวจติดตามตามนัด หรือมีเลือดออกหลังมีเพศสัมพันธ์ เลือดออกระหว่างรอบเดือน หรือตกขาวผิดปกติเรื้อรัง ซึ่งควรตรวจไม่ว่าจะตรวจคัดกรองครั้งล่าสุดเมื่อไร

Few sentences cause more alarm than an abnormal smear result, and most of that alarm is unnecessary. Cervical cancer is one of the few cancers screening can largely prevent rather than merely detect early, precisely because it is preceded by changes that remain treatable for years.

An abnormal result usually means cell changes, not cancer. The common categories run from borderline changes, through low-grade changes which frequently resolve on their own, to high-grade changes which are treated to stop them progressing. Only a small minority of abnormal results represent anything close to cancer.

What happens next depends on the grade and on the HPV result alongside it. Options include repeating the test after an interval, since many low-grade changes clear by themselves; HPV testing to clarify the risk; or colposcopy — a magnified look at the cervix, done in clinic, taking about fifteen minutes, with a small biopsy if something needs checking.

If treatment is needed it is usually a minor procedure to remove the affected area, done as an outpatient. It is highly effective and rarely affects future pregnancy, though that is worth discussing if you plan to conceive.

The single most important thing is not skipping the follow-up. Cervical changes are slow, which is what makes them so treatable — and also what makes it easy to postpone the appointment, and then postpone it again.

Bring the date and result of your previous screen if you have them, because they genuinely change the interpretation. See a doctor promptly if you have had an abnormal result and have not had the recommended follow-up, or if you have bleeding after sex, bleeding between periods, or unusual persistent discharge — those warrant assessment regardless of when you were last screened.
