Is the HPV Vaccine Worth Having After 30?

The 9-valent HPV vaccine is licensed up to age 45, and the question women over 30 ask is a fair one: if the vaccine works best before exposure, and I have been sexually active for years, has that ship sailed?

Not entirely, and the reason is arithmetic. The vaccine covers nine HPV types. Most people who have been exposed have encountered one or two, not nine. Whatever you have not met, there is still something to protect against. That is the honest version — it is a reduced benefit rather than no benefit, and how much it is reduced depends on your history.

What the vaccine does not do is equally important. It is preventive, not therapeutic. It does not clear an existing HPV infection and it does not treat an abnormality already found on a smear. If you have had an abnormal result, vaccination is a separate conversation from managing that result, not a substitute for it.

It also does not replace screening. The vaccine does not cover every oncogenic type, and many adults are vaccinated after some exposure — so cervical screening continues on the normal schedule regardless. Anyone who tells you a vaccinated woman no longer needs smears is wrong.

The schedule is typically two doses if the course is started under 15, and three doses at 15 or older. A course begun in another country can usually be continued rather than restarted, so bring the record if you have it.

Whether it is worthwhile for you specifically depends on age, history and circumstances, and is decided in consultation. See a doctor to discuss it if you are under 45 and unvaccinated, if you started a course and never finished it, or if you are due a screening — the two can often be done in the same visit.
