The Vaginal Pessary: The Most Underused Option in Prolapse Care

A vaginal pessary is a small support device fitted inside the vagina to hold the descended organ in position. It is one of the oldest treatments in gynaecology and one of the most misunderstood.

Three assumptions get in the way. The first is that a pessary is what you are offered when you are too old or too frail for surgery. It is not — it can be fitted at any age, including for women in their thirties. The second is that it is a temporary measure while you wait for an operation. For many women it is a permanent choice, and a reasonable one. The third is that it will be uncomfortable. A correctly fitted pessary should not be felt at all.

That last point is worth dwelling on, because it is where most disappointment comes from. If you are aware of the pessary, it is the wrong size or the wrong shape. Refitting is a normal part of the process rather than a failure, and finding the right one sometimes means trying more than one. Being told it does not suit you after a single attempt is not the end of the road.

A pessary particularly suits women who want more children, since surgical repair is usually deferred until childbearing is complete; women for whom surgery carries additional risk; and women who simply do not want an operation and are entitled not to have one.

Practically: many women remove, clean and reinsert it themselves, which most people learn quickly. Others prefer to have it checked and cleaned periodically at the clinic. It can be removed at any time, and removing it changes nothing permanently — which is exactly what makes it a low-risk thing to try before committing to surgery.

See a doctor if you have a bulge you can feel, if a fitted pessary becomes uncomfortable or falls out, or if you notice bleeding or unusual discharge while using one.
