Why Prolapse Often Appears at Menopause, Years After the Births

A common and confusing pattern: a woman gave birth in her early thirties, felt fine for twenty years, and then in her early fifties develops a heaviness that was never there before. Nothing obvious happened in between. So why now?

Two things are true at once. Pregnancy and delivery stretch the pelvic floor and its supporting tissue, and that reduced support can sit silently for decades because the surrounding tissue is strong enough to compensate. Then, around menopause, estrogen falls — and estrogen is part of what keeps vaginal and supporting tissue thick, elastic and well supplied with blood.

As that tissue thins and loses elasticity, the compensation stops working. The prolapse that was always structurally present becomes symptomatic. This is why the timing feels arbitrary and why women often assume they have done something to cause it. They usually have not.

The same hormonal change explains why prolapse so often arrives alongside other symptoms at the same time — vaginal dryness, discomfort during sex, urinary urgency, and recurrent urinary infections. These are frequently treated as separate complaints by separate visits when they share a single underlying cause and are worth raising together in one appointment.

It also changes what helps. Where thinning tissue is a significant part of the picture, addressing that can make other treatments work better — a pessary is easier to fit and more comfortable in tissue that is in better condition. Suitability is decided individually after assessment.

See a doctor if you notice a bulge or new heaviness around menopause, if sex has become painful, if you have recurrent urinary infections, or if several of these arrived together — the fact that they arrived together is useful information, not a coincidence.
